Evaluation of NCD Coaches and Churches

The Evaluations of Church by Coach and Coach by Church will be reviewed and processed by the Office of Congregational Transformation (OCT). The Director of the OCT will initiate the appropriate action and feed back.

When you are finished recording your evaluation click on the box at the bottom of the form to print the results on your printer. Please sign the printed copy and mail or fax it to the OCT. Keep a copy for your records. These forms cannot be submitted on line.
